The phrenic nerve (C3-C5 root value) provides the sole motor supply to the diaphragm; its injury during cardiac or thoracic surgery (it runs anterior to the root of the lung and along the pericardium) causes ipsilateral diaphragmatic paralysis and paradoxical upward movement on inspiration.
