Unconjugated bilirubin is lipophilic and poorly water-soluble, so it circulates bound to albumin; displacement of bilirubin from albumin (e.g., by certain drugs, or when albumin-binding capacity is exceeded in neonatal jaundice) allows free bilirubin to cross the blood-brain barrier, risking kernicterus.
